session DESCRIPTION
This session will address the less common and challenging situations that clinicians face in glioma management. The classification of gliomas has become more complex, with increased emphasis on molecular features, alongside traditional histological findings. While newer entities have been recognized, optimal management strategies are less well defined, thus posing challenges for clinicians. This situation is further complicated when histological features do not align with molecular features. In nearly all patients, at the time of glioma recurrence, there is no standard management strategy. Reirradiation is recognized as a useful option in select patients, but there remains uncertainty as to how reirradiation is best applied. This exciting and practical session will include presentations from experienced adult and pediatric Radiation Oncologists covering dedicated topics not previously addressed in prior CNS Education Sessions, with an emphasis on practical treatment planning guidance for practitioners. Topics to be covered include molecular glioblastoma and brainstem glioma with a focus on WHO classification updates and potential impact on management, practical considerations for treatment (including target volume definition and dose) and the role of systemic therapies. In addition, the timely topic of reirradiation in recurrent glioma will be comprehensively covered, including advanced imaging for confirmation of recurrence vs. treatment effect, target volume definition and dose, the role of bevacizumab and the latest trial evidence to support best practice. The session aims to equip practitioners with an up-to-date, evidence-based approach for managing these complex and increasingly common situations.
