EDU 08 - Beyond the Bone: Difficult Cases that Stretch the Definitions of Palliative Radiotherapy

session DESCRIPTION

As breakthroughs in oncologic care extend life expectancies for patients with advanced cancer, the borders between palliative- and curative-intent radiotherapy begin to blur. Aggressive palliative radiotherapy has been best investigated in the context of dose escalation for metastatic sites in oligometastatic disease states; however, significant questions remain about optimal care of symptomatic but likely incurable primary disease in patients with anticipated prolonged life expectancy. Notably, strategies used to palliative distant metastatic sites like bone lesions may not be appropriate for managing the primary disease site. This session will use a case-based format to highlight themes critical for the care of patients with symptomatic but likely incurable primary disease that warrants attempt at durable local control. Presenters will address evaluation, treatment and symptom management strategies specific to this clinical context, drawing from published research, consensus guidelines, ongoing clinical trials, and novel applications of existing treatment approaches. They will also address alternative or complementary approaches to symptom management (e.g., ablation, nerve blocks, stenting) when relevant. A goal of this session is to stretch attendees' conception of the scope and application of palliative radiotherapy to match its increasingly expansive definition relative to durable symptom management.

  1. Determine indications for aggressive palliative radiotherapy in patients with symptomatic primary disease sites.
  2. Identify optimal radiotherapy dose, fractionation, and delivery strategies for aggressive palliative management of primary disease, as well as non-radiotherapy alternatives.
  3. Implement strategies to manage symptoms of disease and side effects of treatment in this clinical context.
