EDU 38 - At the Margins: Navigating Challenging Vulvar and Vaginal Cancers

session DESCRIPTION

This case-based session will explore emerging treatment paradigms for the management of vulvar and vaginal cancer. Although relatively rare, the incidence of vulvar and vaginal cancers is rising, and our understanding of how to best treat these diseases has evolved with the recent publication of studies such as GROINSS V-II, GOG 279, and RetroEMBRAVE. There is also increased awareness of the molecular drivers which may impact prognosis and influence treatment approaches. Session speakers will address the latest data regarding the management of node positive and locally advanced vulvar cancer including the appropriate incorporation of sentinel node biopsy, standardized contouring guidelines and treatment techniques for IMRT, and when to consider brachytherapy boost to gross primary disease. Similarly for vaginal cancer, participants will become familiar with the recently published GEC-ESTRO contouring recommendations, and the data driving the increased use of image-guided brachytherapy and the potential benefit of escalating doses for T2-T4 tumors.

  1. Recognize settings in which sentinel node biopsy versus lymph node dissection should be used in node positive vulvar cancer.
  2. Distinguish when locally advanced vulvar patients should be managed with neoadjuvant versus a definitive radiation approach +/- brachytherapy boost.
  3. Implement standardized treatment volumes and image-guided brachytherapy for vaginal cancers.
