EDU 39 - Handling CNS Metastases in Sick Patients

session DESCRIPTION

Management of CNS metastases requires nuance with respect to patient performance status, comorbid conditions, extracranial disease status and concurrent systemic therapies. Situations become more complicated for inpatients at end of life, when even a 5-fraction course could delay discharge to hospice. Acceptable options may vary from 1-2 fractions of whole brain RT to SRS for dominant lesions to no RT at all. Our panel will review these options in detail, with the decision process associated in each case. Dr. Hoskin will discuss situations where no CNS-directed therapy may be defensible and, in the patient's best interest. Dr. Keit will discuss data supporting 1-2 fraction courses of WBI in appropriate populations. Dr. Soliman will discuss a directed option of SRS to dominant lesions.
