EDU 03 - Nonoperative and Preoperative Local Management of Breast Cancer: From Data to Shared Decisions

03:00pm - 04:00pm ET

MODERATOR(S)

Sachin Jhawar, MD, MS - The Ohio State University James Cancer Hospital

session DESCRIPTION

This session will explore emerging strategies for preoperative and nonoperative local management of breast cancer, focusing on when and how radiotherapy or ablative techniques might safely substitute for or reshape surgery in select patients. Topics will include: (1) current evidence and ongoing trials of preoperative partial breast RT/SBRT, including immune effects and implications for pathologic response; (2) nonsurgical management after neoadjuvant systemic therapy in patients with image guided biopsy-confirmed exceptional response, and frameworks for considering omission of surgery; and (3) definitive nonsurgical local options for inoperable patients or those who decline surgery, including SBRT and cryoablation. Case based discussion will highlight real world implementation, shared decision making, and communication of uncertainty with patients considering nonoperative pathways.

learning objectives

  1. Describe current evidence and key ongoing trials evaluating preoperative partial breast RT/SBRT and their implications for pathologic response and treatment volumes.
  2. contribute to nonoperative management decisions with patient values and equity considerations.
  3. Compare nonsurgical local options such as SBRT and cryoablation for patients who are medically inoperable or decline surgery, including expected local control, toxicities and patient reported outcomes.
