session DESCRIPTION

The role of radiation therapy for rectal cancer has evolved rapidly in the last four years, with the publication of four major randomized trials, as well as widespread media attention. Given these changes, the ASTRO Rectal Cancer Guidelines Panel developed a focused update to the 2020 ASTRO Rectal Cancer Guidelines. This session will present the key recommendations from the focused update, using a case-based interactive format and Audience Response System to illustrate different clinical scenarios. Specifically, the session will discuss 1) Patient selection for when radiation therapy may be omitted, 2) Radiation therapy as part of total neoadjuvant therapy, including discussion of long course vs. short course radiotherapy, and discussion of radiation and chemotherapy sequencing options, 3) Role of radiation therapy as part of non-operative management (watch and wait). The session will be of widespread interest to both community and academic radiation oncologists and will help radiation oncologists effectively collaborate in the multidisciplinary management of this common disease.

learning objectives

  1. Identify patients with rectal cancer for whom radiation therapy can be omitted based on clinical and imaging-based risk factors as well as biomarkers.
  2. Select the optimal total neoadjuvant therapy regimen for patients with rectal cancer, with the radiation regimen (long course or short course) and treatment sequence (radiation first or chemotherapy first) tailored to the specific patient.
  3. Use radiation therapy as part of non-operative management (watch and wait) in patients with rectal cancer.
